Our cardiopulmonary rehabilitation program helps patients with fitness and function following a cardiac or pulmonary event.
You may qualify for cardiac or pulmonary rehab if you have never done rehab for your diagnosis before and if you have had any of the following:
Our expertly trained staff will personally supervise your rehabilitation program, help you determine activities appropriate to your condition and provide personal encouragement, education and motivation to help you meet your goals.
Your support team may consist of doctors, nurses, exercise specialists, dietitians and mental health specialists.
We will work with you at one of our outpatient facilities in Clinton, Osceola or Warsaw to create a customized program to meet your specific needs that includes exercise training, education and counseling on heart-healthy living.
By working together, we can improve your health and quality of life, reduce the need for medicines to treat heart or chest pain, decrease the chance you will go back to a hospital or emergency room for a heart problem, prevent future heart problems and even help you live longer.

Our Phase II cardiopulmonary rehab services will provide you a structured exercise and education program to help you with fitness and function following a qualifying cardiac event. Typically, cardiac rehab is ordered upon your discharge from the hospital or may be ordered by your primary care provider. However, patients can also self-refer. Most patients will do 36 visits, attend three days a week and exercise for 45 minutes.
In most cases, this program is a reimbursable service under Medicare, Medicaid and most private insurance companies with a qualifying diagnosis.
Our Phase III program is an extension of Phase II with an emphasis on helping you maintain a healthier lifestyle and further achieve your goals through self-structured exercise sessions. Those who do not have a qualifying diagnosis for insurance can often utilize this program.
Phase III is a self-pay program and is similar to a gym membership.
