Our Home Health and Hospice departments provide skilled care for our patients and their family members within their home setting, with licensed nurses on call 24 hours a day.
Our home health agency provides assistance to you or your loved one during recovery from an illness, surgery, injury or long-term medical problem in the privacy, comfort and security of your home. Skilled nursing, behavioral health services and therapy (physical, occupational, speech) services are delivered in the home with the goals of improving health, regaining independence, becoming as self-sufficient as possible and/or maintaining the current condition or level of function.
The home health program intends to improve quality of life and increase the patient’s strength, endurance and functional abilities. To qualify for home health services, a patient must meet the following criteria:
Our home health professionals have been recognized for their exceptional commitment to the patient experience. In 2019, they were named a Top Agency of the 2019 HomeCare Elite®, a recognition of the top-performing home health agencies in the United States. The home health team also received a five-star rating, on a one-to-five scale with five stars being the best, from the Medicare.gov Home Health Consumer Assessment of Healthcare Providers and Systems, which measures the experiences of people receiving home health care from Medicare-certified home health agencies.
Our Hospice Services are a compassionate, specialized healthcare service designed to care for terminally ill patients and their families at home or in a nursing facility.
